Innovaccer Acquires Story Health to Pioneer AI Agents that Augment Specialty Care Teams

Sep 16, 2025

AI-powered, tech-enabled platform focused on transforming specialty care from episodic to continuous, digital models. This acquisition builds on Innovaccer's strategic acquisitions including Humbi AI, Cured, and Pharmacy Quality Solutions, extending its Healthcare Intelligence Cloud into agentic care augmentation. Over the past years, Innovaccer has built the foundations of a fully integrated healthcare operating system from data unification to patient engagement to pharmacy quality, actuarial intelligence and, now, agentic care augmentation. Founded in 2020 by Google veterans Tom Stanis, Nikhil Roy, and Cardiologist Ashul Govil, MD, MBA, Story Health brings a vertically integrated cardiovascular care platform designed to shift care from episodic visits to continuous management digitally. By combining AI-driven clinical pathways, advanced medication workflows and human-led coaching, the company has delivered industry-leading results across heart failure, hypertension and Atrial Fibrillation (AFib). Health systems using the platform have seen a 6.9% 30-day HF all-cause readmission rate (compared with the national average of 18.1%), more than 60% reductions in hospitalizations, and an overall improvement in daily engagement and medication adherence. With Story Health, Innovaccer will pioneer agentic care augmentation, where AI agents assist specialty care teams with routine, non-clinical tasks and provide continuous patient engagement between visits. Health coaches, working alongside AI agents, can converse directly with patients about symptoms and medication adherence, monitor data from remote patient monitoring devices, and trigger timely interventions. By embedding this model into the EHR and existing workflows, Innovaccer will help health systems scale specialty care capacity, reduce hospitalizations, and improve adherence without increasing clinician burden. “Healthcare doesn't change through dashboards alone,” said Abhinav Shashank, cofounder and CEO of Innovaccer. “It changes when data and AI power completely new clinical models. Story Health has proven that in specialty care; and we're excited to bring this technology and clinical expertise to our health system customers nationwide.” Tom Stanis, cofounder and CEO of Story Health, added, “When we built Story Health, we believed specialty care needed more than software, it needed an operating model. Most platforms stop at insights. Story Health goes all the way to action. Joining forces allows us to scale nationally and turn continuous, AI-assisted specialty care into the new normal.” The acquisition further strengthens Innovaccer's Healthcare Intelligence Cloud and moves the company towards scalable clinical transformation, making continuous, AI-enabled specialty care possible for providers and health plans across the U.S. “Story Health has helped us reimagine how we deliver cardiac care at ChristianaCare. By extending care beyond the walls of our clinics and into the daily lives of our patients, we've seen improvements in medication adherence, fewer hospitalizations, and stronger engagement between visits," said Kirk Garratt, MD, Medical Director of the Center for Heart and Vascular Health and Chair of Cardiology at ChristianaCare. "The integration with Innovaccer will allow us to scale this model further, empowering our teams to provide continuous, personalized care without adding to clinician burden. This is a critical step forward in making advanced, AI-enabled specialty care accessible to more patients in our community.” Existing Story Health customers will continue to receive uninterrupted service, with expanded access to Innovaccer's broader AI-powered capabilities. Story Health continues its commitment to solve the need for continuous care across other specialties and will be launching Diabetes and COPD. The combined platform will support fully integrated clinical pathways across primary and specialty care, empowering health systems to operate with new levels of scale, speed, and clinical precision.
