11일 서울 양재 엘타워에서 열린 '메드텍 인사이트(Medtech Insight) 2025' 콘퍼런스에서 미국 의료기기 시장의 구조와 진입 전략을 놓고 구체적 해법이 공유됐다. 한국 기업이 미국 시장에 도전할 때 △FDA 승인 △보험 등재 △KOL(Key Opinion Leader) 네트워크가 핵심이라는 설명이다. 토마스 구스타프손 아널드앤포터 수석 정책자문은 FDA 승인과 보험 등재 차이점에 대해 “FDA는 제품이 '안전하고 효과적인가'를 기준으로 시장 진입을 허가한다. 하지만 정부와 보험사는 '치료에 합리적이고 필요한가'를 판단한다”면서 “FDA 승인이 곧 환자 사용과 수익으로 이어지는 것은 아니다”라고 말했다. 즉 FDA 승인은 마트에 진열할 자격을 얻는 것이고, 보험사는 환자가 실제로 '장바구니에 담을 때' 비용을 지불할지 결정한다는 것이다. 구스타프손 수석 정책자문은 메디케어·메디케이드 서비스 센터(CMS)와 민간 보험사들이 요구하는 데이터에도 차이가 있다고 설명했다. 그는 “FDA는 임상시험에서 안전성과 효능을 입증하면 되지만, CMS는 실제 진료 현장에서 환자에게 효과가 있는지까지 본다”면서 “실험실 데이터가 아니라 현장 데이터가 필요하다”고 말했다. 이어 “기존 제품 대비 개선 효과를 객관적 데이터로 보여주지 못하면, 아무리 혁신적 기술이라도 더 높은 보험 수가를 받기 어렵다”고 덧붙였다. 또 보험 코드(CPT·HCPCS)와 급여 체계의 복잡성도 짚었다. 그는 “코드가 있다고 자동으로 급여가 되는 게 아니다. 기본 코드로 진입할 수는 있지만, 고가 신기술이라면 별도의 코드나 신기술 보상 프로그램을 활용해야 한다”고 전했다. AI와 관련해선 “AI를 넣는다고 무조건 가치를 인정받는 게 아니다”면서 “임상적으로 어떤 차이를 만들었는가를 증명해야 한다”고 말했다. 알렉산더 팍스투자크 유타대 교수는 의료기기 상업화에서 KOL의 중요성을 강조했다. 그는 “FDA 승인과 병원에서 채택은 다르다”면서 “실제 도입은 병원 가치평가위원회와 현장 의사들의 판단에 달려 있다”고 말했다. 이어 “특히 지역 병원이나 외래 수술센터에서는 영향력 있는 의사의 한마디가 구매 결정을 바꾼다”고 전했다. 미국 시장 진입은 FDA 승인보다 KOL 네트워크와 병원 가치평가위원회 설득이 중요하다고 강조했다. 미국 의료시장의 다층적 구조도 설명했다. 연방 차원에서 구매하는 보훈병원(VA)·국방부 병원은 긴 구매 절차와 엄격한 기준을 두고, 대학병원은 임상 데이터와 경제성 분석을 중시한다. 반면 지역 커뮤니티 병원과 외래 수술센터(ASC)는 비용과 수익성을 더 우선시해 결정이 빠르다. 이 과정에서 KOL의 존재가 결정적이다. 팍스투자크 교수는 “한국 기업은 초기 단계부터 글로벌 학회와 임상 네트워크를 통해 KOL과 장기적 관계를 구축해야 한다”고 조언했다. 미국 규제 환경에 대해 팍스투자크 교수는 “의사가 기업에게 금전적 대가를 받고 처방을 늘리는 행위는 선샤인 액트(Sunshine Act)에 따라 단순 강연료나 자문료도 모두 공개된다”면서 “한국 기업이 진출할 때는 윤리적이고 투명한 방식으로 KOL과 파트너십을 맺어야 한다”고 설명했다. 송혜영 기자 hybrid@etnews.com
